Applyling Online

So there's this job at a restaurant that's just opening up in April that I want to apply to. I noticed they have an Apply Online choice on their website, and I'm thinking that's the only way to apply since I don't think the place is open yet. Or how else I would submit my resume.
So basically, I have to fill in all these questions, all that would pretty much be on my resume but some of them I'm not sure how to answer:
MY WORK HISTORY

Please list your work history, beginning with most current.

* Annual Base Salary
* Annual Bonus:

Can they ask me that? I don't know what my salary was.. I worked part time. I didn't keep track.

Also,
Here is your chance to impress us! Why should we hire you?
I think I'm going to mention my volunteer experience. Anything else that would sell me??

Also, if anyone knows how to apply to a place that isn't open yet, other than online, please let me know.

I think online is the best way-I've applied for jobs online and gotten them before. In retail they usually like you to drop it off, but if the place isn't open yet there won't be any other way to easily get it to them.

As for your salary, just write down your hourly wage (like put 8.50/hr or something like that). And yes, places can ask that (although they usually ask how much you expect to make and most people will put down what they make at their current job).

Volunteer skills are good, but also include any qualities you have that make you good for the job (you know, the general stuff like hard working/problem solver/etc) along with that. Don't just mention your volunteer stuff but say what you got out of it (like learning to work in a high pressure situation or leading a group or something like that).

Okay, so for the salary question, it's a scroll down list with options 25k, 30k, 35k, 40k and 40k+ So should I just answer 25K since it's the closest to what I made??

Also, the title of that bit is: Annual Income, Target Starting Base Salary
So Target Starting Base.. meaning what I plan on making??

And, this is just a resume question, but when they say education, do I put my highest COMPLETED education (highschool) or as far as I've gone (University, still in it)?

Ah, so many questions, but one more:
So it asks for my reason for leaving, and I don't really have a good reason. Ideas of what I should put??

Hmm...I'm really not sure about the salary question so I'll just let someone else answer that.

But for the education, I'm almost certain that you put down as far as you've gone like if you're in university, write what you're majoring in (at which school) and which year you're currently enrolled in (first, second, third, fourth year.)

And for the reason for leaving, I'm not sure if your current job is in another focus area (e.g. retail) or not...but if it is, you could always write that you want to experience or focus on a different area of work (in this case, foods industry.)

So for changing jobs, I'm going from food to food.. but I guess I could say how I want to get into waitressing, rather than just serving (Tim Hortons) or something like that??

Yeah, I guess you could say that you want to be part of the food industry from a different view, being able to communicate more directly with the customers.

Maybe say you want to move away from like, something more fast-food oriented?

And yes-you ALWAYS put the level of education you're currently at. So like, you'd put you went from high school from 2001-2006. Then for uni you'd put 2006-present.

I'm not totally certain...but you should probably put 25k, which is what you're closest to making. Or you could always send in an email to them asking about it and telling them of your situation.

One more question.
Any idea of how I could do a followup for this job? The website doesn't have a Contact Us feature, so would I not be able to do this and just have to wait and see what happens?

And I didn't want to start a new topic since it's kinda about applying online. If I apply to a job online by email, is it okay if I hand in my résumé in person as well? Or would it be totally unneccessary?
